Dr Graham Edkins is a highly regarded international safety specialist with more than three decades of experience designing, implementing, and reviewing Critical Risk Management (CRM) programs for organisations operating in complex and high-hazard environments. His career spans executive leadership, safety regulation, accident investigation, and advisory roles across mining, transport, aviation, utilities, construction and government sectors. He is recognised for helping organisations strengthen their capacity to prevent fatalities, serious injuries and catastrophic events through practical, governance-driven critical control systems.
Dr Edkins commenced his career with the ATSB, where he led major aviation accident investigations and later joined Qantas Airways as Head of Human Factors. During this period, he began his PhD under the supervision of Professor James Reason, one of the world’s foremost authorities on organisational accidents. Dr Edkins subsequently held senior executive and regulatory appointments including Executive Director of Public Transport Safety Victoria (PTSV) and Group General Manager roles at the Civil Aviation Safety Authority (CASA).
Dr Edkins currently advises boards, chief executives and operational leaders on enterprise Critical Risk Management (CRM), fatality prevention and human factors programs that strengthen governance and provide assurance over major hazards.
An internationally recognised authority in human factors and organisational performance, Dr Edkins specialises in understanding how leadership, organisational systems, work design and human performance influence critical control effectiveness. More recently, he has assisted organisations to integrate psychosocial hazard management into enterprise safety systems and systemic investigations.
Dr Edkins has led numerous independent investigations into fatalities, multiple-fatality events and other high-consequence incidents across a range of high risk industries. He is also an experienced independent expert witness, providing specialist opinions for Coronial Inquests, civil litigation and regulatory proceedings in human factors, safety management systems and psychosocial hazard management.
