🚨 Causal Investigation Report Released: Perilya Mine Fire 🚨

The NSW Resources Regulator has published the Causal Investigation Report into the underground fire that occurred on 12 January 2025 at the Perilya Southern Operations in Broken Hill, NSW. 


The incident, which originated at a previously mined section of the haulage shaft above level 23 (about 1,000 metres below surface), resulted in the emergency evacuation of 46 workers, temporary entrapment of 5 personnel in a fresh air base, and a complete halt to operations for an extended period. 


Although no injuries were reported, the event exposed critical failures in planning, contractor oversight, hazardous chemical identification, and risk management.
